REGISTRATION FORM

'Game On' Camp Parent & Player Information

The camp is free to those associated with Special Olympics.
Players are encouraged to wear comfortable tennis shoes (no open-toe), shorts and a short-sleeved T-shirt (NHL Street jersey will be provided free of cost via registration)
Water will be provided and medical personnel will be on staff. Players are encouraged to bring their own water bottle
The camp consists of a half-hour of skills stations and a half-hour of street hockey games
All equipment, including sticks, balls, nets and goalie equipment will be provided. Players will be able to keep their NHL Street jersey, but equipment stays with the camp for future players
This format allows kids of all skill levels to enjoy the fun and excitement of NHL STREET, no matter their previous experience in hockey
